Every year, thousands of children are born with a cleft palate, cleft lip or a combination of both. The advocacy group AmeriFace promotes the month of July as National Cleft & Craniofacial Awareness & Prevention Month to call attention to this potentially disfiguring defect—and to highlight treatments offered by dentists that can change the destiny of a child with a cleft defect.
Simply put, a cleft is a gap or opening in the palate (roof of the mouth) and/or upper lip. Cleft lips and palates result when structures that are forming in an unborn baby’s mouth and face don’t fuse together as they should during pregnancy. They can occur on either one side or on both sides of the face, in partial form (with some connecting tissue present) or completely open.
Clefts can cause severe disfigurement in a child, which may lead to a diminished self-image and disruption in relationships with others. A cleft can also compromise other aspects of a child’s health and life, including dental health, nutrition, respiratory function and speech development.
Doctors don’t always know why a particular baby is born with a cleft lip or palate, but clefts are thought to result from a combination of factors. Genetics most certainly plays a role, but there appear to be other influencing factors during pregnancy like nutritional deficiencies and fetal exposure to alcohol, radiation or toxic chemicals. In addition, having poorly controlled diabetes or being obese during pregnancy may increase the risk of the baby being born with cleft lip or cleft palate.
Managing known health conditions as well as striving for better prenatal nutrition and protection from environmental hazards may reduce the risks for cleft formation, even so, clefts do form. When they do, we can often effectively correct them, thanks to surgical procedures first developed by a military surgeon stationed in Korea in 1950.
While analyzing photos of cleft patients, Dr. Ralph Millard realized the tissue needed to repair a cleft was already present, but in a distorted form. He then experimented with surgical techniques that released the tissue so that it could be moved and fashioned into a normal appearance.
Dr. Millard’s original techniques remain the basis for today’s advanced procedures. Correction of a cleft lip or palate typically requires a series of procedures which can span the child’s developmental years. The first surgery usually occurs around 3-6 months of age, followed by later procedures to refine the earlier work. This process usually requires a team of dental specialists that includes oral surgeons, orthodontists and general dentists.
The road to restoration from a cleft birth defect can be a long one for children and their families, but the treatment methods developed over the last several decades can truly give them the gift of a normal life.

While wearing braces is the path to a healthier and more attractive smile, it can be a difficult journey. One of your biggest challenges will be keeping your teeth clean to avoid a higher risk of tooth decay.
Tooth decay starts with dental plaque, a thin film of bacteria and food particles that accumulates on teeth. Daily brushing and flossing clear this accumulation. But the hardware of braces makes it difficult to access all tooth surfaces, and can even become a haven for plaque.
One sign in particular of tooth decay while wearing braces is the appearance of chalk-like spots on the teeth known as white spot lesions (WSLs). WSLs occur because the minerals in the enamel beneath them have begun to break down in response to decay. The spots can eventually cause both structural and cosmetic problems for a tooth.
The best approach to WSLs is to prevent them from developing in the first place. You'll need to be extra vigilant with daily oral hygiene while wearing braces to reduce plaque buildup. To help with the increased difficulty you might consider using a special toothbrush designed to maneuver more closely around orthodontic hardware. You may also find using a water flosser to be a lot easier than flossing thread.
Preventing tooth decay and WSLs also includes what you eat or drink to reduce the effects of enamel de-mineralization. The bacteria that cause decay thrive on sugar, so limit your intake of sweetened foods and beverages. And to avoid excessive demineralization cut back on acidic foods as well.
If despite your best preventive efforts WSLs still form, we can take steps to minimize any damage. For one, we can give your enamel a boost with fluoride applications or other remineralization substances. We can also inject a tooth-colored resin beneath the surface of a WSL that will make it less noticeable.
With any of these and other treatments, though, the sooner we can treat the WSL the better the outcome. Practicing good hygiene and dietary habits, as well as keeping an eye out for any WSL formations, will do the most to protect your new and improved smile.

A certain news story a few years ago notwithstanding, dentists still recommend flossing along with brushing to adequately remove disease-causing plaque. If there is any controversy at all about flossing, it's whether you should perform it before brushing or after. Each perspective has good reasons.
"Brush First" proponents say their way encourages the formation of a daily hygiene habit, a must for preventing disease. That's because brushing can remove most of the plaque built up on the teeth, while flossing can then remove what's left. If you floss first, though, you'll have to plow through the sticky film with the floss, which can be an unpleasant experience. Facing that every day could make a person less enthusiastic about developing a hygiene habit.
But it's not just about the sensation: depending on the person, the plaque buildup could be so much that the floss becomes clogged with it. You're then moving the plaque rather than removing it. Brushing a lot of the plaque out of the way first will increase the cleaning power of your floss.
The "Floss First" team, though, is undaunted with their own take on the matter. Flossing can loosen up any stuck debris between teeth, making it easier for brushing to clear it away. It can also expose plaque-covered areas between teeth to allow better contact with the fluoride in your toothpaste. And, the amount of plaque you're pulling out in certain areas during flossing could tip you off to beef up your brushing efforts on those areas of heavier plaque accumulation.
One of the prime reasons for flossing first, though, goes back to the comfort factor and human nature. To be honest, for most people flossing isn't as much "fun" as brushing. If you put it off until after brushing, you're more likely not to do it if you find it unpleasant. Doing it first gets the less likeable task out of the way, so you can then do the more likeable one, brushing.
Which approach is best for you? It's really a personal decision, one you can discuss with your dentist. Try both ways, and see which one seems better. But whether you floss first or last, do floss to really reduce your risk for dental disease.
